Service Description

If you need help managing your medication, Dreams of Hope Foundation can help you. This program connects individuals to resources and teaches life skills to ensure medication is taken as prescribed.  One of our staff members will personalize your schedule and stay in constant contact with you regarding your medication schedule.  Assistance with paying co-pay can be obtained on an individual basis. This service will also refer to providers for medication management.
